This is a service where an accredited Pharmacist, such as Amanda, comes to your home to talk to you about your medicines and any problems or uncertainties you may have.

The Pharmacist will then discuss the results with your doctor with the aim of improving your health. This service is fully funded by the Federal Government for qualifying patients. Any Australian citizen over 65 years or has a chronic disease such as a diabetes, is eligible for a HOME MEDICINES REVIEW.

If you take 5 or more medications on a regular basis, have been recently discharged from hospital or are having difficulties with side effects or managing your medications then talk to your doctor, or one of our Pharmacists about a referral for a home medicines review.
